Deer Creek High School Guest Dance Form

Deer Creek High School Dance Rules and Guidelines:

• Only one (1) guest will be permitted per DCHS student.

• DCHS student must provide a current ID in order to enter the dance.

• Guest must provide a current ID in order to enter the dance.

• Guests must be currently enrolled in a high school (public or private) and is high school age.

• Guests will not be permitted to enter the dance without a properly completed and submitted guest dance form. MUST BE SUBMITTED 8 DAYS BEFORE THE DANCE OR THERE IS A $10 FEE.

• There are no re-entry privileges at the dance.

• Any person found under the influence of and/or in possession of illegal substances will be removed from the dance and turned over the police.

• DCHS enforce the rules of conduct and may ask students to leave the dance if rules are not followed. Parents and school administrators of guest and DCHS student will be contacted for serious violations, and further actions will be taken as necessary.
